Every provider says they are person-centred. These are the specific, checkable things we do differently — the ones you can hold us to at a plan review.
Four to six residents per residence. It is the single biggest determinant of whether staff genuinely know the people they support.
Support plans written in plain language with measurable goals, shared with families quarterly — including where progress has stalled.
A clear yes, a place on the list with an honest outlook, or a no with alternatives. We will not keep you hopeful to keep our list long.
Nobody works independently until orientation, mandatory training and person-specific protocol training are complete and signed off.
Restrictive measures only where necessary to prevent serious harm, always consented, always with a written plan to reduce them.
Every family has a named coordinator. You should never have to chase us for news about someone you love.
How it works
Response within 2 business days
Tell us who you are supporting and what you are looking for. A phone call is fine; so is the form.
1–3 weeks, depending on records
We gather the assessments, plans and funding information needed to understand the situation properly.
2–4 weeks
We meet the person, visit where they live now if that helps, and complete an assessment of needs and goals.
Within 2 weeks of assessment
We give you a clear answer: an offer with a start date, a place on the waiting list, or an honest decline.
Planned over 2–8 weeks
A staged move, with visits before the move date, staff trained on the plan, and a settling-in review.

Homes and community programs throughout Toronto, Peel, York, Durham and Halton. Just outside our area? Ask anyway — we will tell you honestly whether we can serve you well, and point you elsewhere if we cannot.
